Position Overview Under the supervision of the Homebase Assistant Program Director, the Intake Case Manager will coordinate intake and referral process for adult diversions at 30 th Street Intake Center, Franklin Women s Center and Riker s Island. Job Functions : Screens and assesses applicants applying for HomeBase Services. Serves as the primary initial SUS contact point for HomeBase applicants Navigates applicants for program services through intake process, documentation standards and necessary supports. Conduct intake and follow-up appointments with program applicants in person, and if necessary in the field at Department of Homeless Services (DHS) installations, homeless shelters and other outreach/recruitment locations. Understand and communicate the programs eligibility and admission criteria. Demonstrate sound understanding of HomeBase Assess households immediate service needs, providing referrals when necessary for immediate services. Document intake screenings and maintain records for each applicant and enrolled participant in DHS CARES. Collect required documentation for program enrollment and scan documents for digital archiving. Assist applicants with obtaining necessary financial documentation for program enrollment. Participate in team meetings. Prepare formal letters and enclosures for distribution to applicants and referral sources. Completion of other tasks as assigned by manager. Additional duties as assigned by program leadership. Bachelor s Degree in Human Service or related field or High School Diploma or GED with a minimum of 2 years applicable experience Working knowledge of homelessness, substance abuse, criminal justice, HIV/AIDS Strong computer skills and knowledge of Microsoft Office applications.
